Help Identifying ball screw :(

so i have been trying to identify what type, size, and manufacture of the ball screw that came with my KDN x2 mini mill conversion kit. KDN is no longer in business, so if someone can help me out here it would be greatly appreciated. I currently extend my Y axis table by 2" and need a 11" ball screw to replace my 9". from what i know, its a 5/8, and uses a nook ball nut with .1250 ball bearings. here are pictures to help thanks in advance
